At Ambassador, we help you navigate government paperwork and coordinate home care services so that you can continue to live in your own home and receive in-home care. For you to become an Ambassador Home Care client and receive home care services, you must qualify for the Community HealthChoices (CHC) waiver program. Eligibility requirements include a level of care assessment and a determination of financial eligibility. Our experienced staff will guide you through the enrollment process and ensure that you receive the highest quality of care to stay happy and independent at home.
You must be a resident of Pennsylvania and a U.S. citizen or qualified non-citizen with a social security number. You must be at least 21 years of age and be income and resource eligible. You must also meet the required level of care for a nursing facility.
Maximus is the Medicaid enrollment broker that conducts the initial assessment to see if you qualify for the waiver program. The first two forms you will need to fill out are PA IEB Referral Form and Physicians Certification Form. Ambassador helps our clients through every step of the process that’s required to qualify for the CHC waiver. This includes the initial referral to Maximus.
During the Maximus enrollment process, you must choose either Keystone First Community HealthChoices (CHC) or AmeriHealth Caritas. Ask the case manager to assign Ambassador Home Care as the agency for home care services (PAS and respite services).
At Ambassador, we recognize that not everyone who requires home care services will qualify for the waiver program. We offer private pay home care plans, for a reasonable fee, for those who require a qualified caregiver and who are not eligible for our waiver programs. Please call our agency staff, who can provide you with information about our flexible pricing plans. We strive to provide highly qualified services within your budget so that you and your loved ones can get the assistance you need without ‘breaking the bank.’
